 Position: Independent Living Coordinator

Full-time, 40 Hours per week                                                                             
Pay Rate: $20 per hour

Southeast Center for Independent Living (SCIL) is a non-profit organization that services individuals with disabilities in the Greater Fall River and New Bedford areas. We provide skills training, advocacy, information and referrals, transition and prevention services, and peer counseling to individuals who wish to become more independent in their life.
Summary

To educate and train the general community in respect to disability issues, emphasis on housing, transportation, health care, personal care assistance and other independent living needs. Provide information and referral, skills training, advocacy, housing, and peer counseling to individuals with disability. SCIL staff shall have a respect for disability issues and the independent living philosophy. Work with staff involved in transition, prevention, and options counseling.

Essential Duties and Responsibilities: (other duties may be assigned)

·         Provide the five core services SCIL offers to individuals with disabilities: skills training, peer counseling, information and referral, advocacy, and transition and prevention necessary for the attainment of Independent Living goals.

·         Develop, coordinate, and implement outreach, education, advocacy, skills training, peer counseling, and information and referral to persons residing in institutional settings such as nursing homes or shelters that wish to relocate into a lesser restricted environment. 

·         Responsible for intake and assessment. Assist in the development and fulfillment of consumers Independent Living Service Plans and monitor goal achievement. 

·         Responsible for long term support services for the Commonwealth Care Alliance (CCA) contract obligations as a Long-Term Support Coordinator for assessments, community support plans and service coordination for One Care members. Provide information and referral and service coordination between providers and individuals with disabilities.

·         Be responsible for quality assurance and performance standards required for SCIL.

Qualifications: 

·         Must demonstrate knowledge of disability issues and experience working within human services in a direct relationship with individuals served. 

·         Must have knowledge and/or experience working with individuals with a multitude of disabilities including behavioral health, substance abuse, cognitive, hearing and vision, and physical disabilities.

·         Must have valid driver’s license and reasonable transportation means.

·         Must have experience with data entry and documentation of services provided using SCIL’s documentation system.

  • Bi-lingual, Spanish speaking a plus 
 
Education and/or Experience 
 
·         Bachelor’s degree in social work, human services, sociology, psychology, or related field. This requirement may be substituted for five years’ experience in human services.
