We manage over 12,000 apartments throughout the United States.  POAH Communities has 450 team members and maintains offices in Kansas City, Boston, Cincinnati and Chicago.  POAH Communities is seeking a highly qualified individual to serve as a Case Manager for Flat 9 at Whittier.

RESPONSIBILITIES
Reporting to the Community Impact Manager, the Case Manager will be responsible for empowering and supporting residents in the following ways:
  • Maintain a caseload of 30 families who live at POAH’s Flat 9 property in Roxbury or are temporarily relocated in Boston-area apartments through the Boston Housing Authority’s CHOICE Neighborhoods Grant.
  • Work 1-1 with residents to assess their needs through detailed assessments, documentation and data tracking
  • Respond to individual and family needs by connecting residents to supportive services in these outcome areas: housing, education, youth, employment, financial stability and community engagement
  • Develop and maintain knowledge of local resources such as food assistance, emergency rent and utility assistance, health insurance and affordable healthcare, accessible transportation and other critical services
  • Coordinate the delivery of services with local human service providers in the greater Boston area
  • Track and measure all case management efforts in a designated database
  • Collaborate with the Community Impact Manager to identify promising programs or opportunities for youth and families in the community, examples include: afterschool and summer programs, career coaching and job training, budgeting workshops, health benefits enrollment and workshops, community engagement activities, and more
  • Effectively and creatively communicate with residents (such as by text, email, newsletter, flyer, bulletin board, door knocking etc.,) to ensure residents are informed of available resources and programs
  • Maintain all necessary information regarding services to residents in a confidential manner following the regulatory guidance provided by HUD. Remain updated on Fair Housing, mandated reporting and other pertinent HUD rules and regulations
  • Collaborate and coordinate with Property Management team to ensure residents remain stably housed and in compliance with lease terms.

QUALIFICATIONS
Qualified candidates will ideally have:
  • A friendly, outgoing, and collaborative mindset when approaching this work
  • Knowledge of Boston-area supportive services, including health, education and municipal services
  • College degree related to social work, urban planning, public health, human services, or community development is preferred but not required. Minimum of one year prior experience in resident/social service programs (preferably in affordable housing)
  • Ability to work with and motivate diverse resident populations
  • Excellent verbal and written communication skills and basic computer skills
  • Ability to travel throughout the neighborhood and engage in door knocking efforts
  • Basic proficiency in the use of computers including Word, Outlook and Excel

Salary is competitive and commensurate with experience. Please note that this position is grant-funded and subject to the possibility of the position ending at the end of the grant period.
