The RAM/WP (Resident Advocacy, Mobility, and Wellness Program) helps residents of JPNDC properties build income, assets, and financial capability through individualized coaching, referrals, and resource navigation. The Sr. Case Manager will support RAM/WP by offering in-depth case management services to clients and preparing them to join the RAM/WP program.
About this Position
The Case Manager will:
- Report to the Associate Director on a weekly basis
- Recruit program participants through various methods, including group presentations, flyering, events, and networking
- Work with each participant to complete a case management Individual Training and Services Plan
- Help participants set and achieve case management goals
- Support RAMWP Mobility Coaches with client case management needs
- Help participants transition into the RAMWP Program
- Build partnerships with service providers and make referrals as needed
- Enter notes on interactions with participants and outcomes achieved in a shared database
- Report on program outcomes, keep records of interactions, enter data, clean data, and identify issues affecting program operations
- Coordinate community-building events with residents
- Provide occasional group workshops
- Perform clerical tasks such as photocopying, scanning, filing, faxing, printing, and logging
- Work from multiple job sites and meet participants in their homes, if needed
What It Takes to Be Successful
- 3-5 years of relevant case management experience
- Bilingual in Spanish
- Associate or bachelor's degree in social work or equivalent work experience
- Experience working in a diverse environment
- Experience with Salesforce, Bonterra, or other CRMs
- Experience planning and executing community events
- Reliable transportation
- Familiarity with presentating to groups of people
- Proven knowledge of local community resources
- Availability to work nights and weekends
- Strong mediation, verbal, and written communication skills
- Strong organizational skills and attention to detail
- Strong problem-solving and decision-making skills
- Ability to work independently and achieve timely completion of assignments
- Proficiency in Microsoft Suite, Word, Excel, and PowerPoint
Work Environment
This is a full-time role with a flexible schedule, including some nights and weekends, and frequent site visits.
Employment Terms/Compensation
The salary for this position is $55,000-$65,000, based on experience. This is a full-time, salaried position with a competitive benefits package.
